  • I understand that if elected, I agree to complete a conflict of interest statement.

  • I understand the time commitments required with being a Board Member of the ITN, including, but not limited to, ITN board meetings, ITN Annual Meeting, board retreats, etc. 

  • I understand that if elected, I agree to submit a photo to the ITN staff members for nomination notifications and other external communications.
